DENTAL BENEFITS

Sele-dent is the provider network used by the plan. If you go out of network, you will only be reimbursed the network amount and you will be responsible for any differences. To find a participating dentist, visit www.Sele-dent.com or call 1 800 520 3368

OPTICAL BENEFITS

Vision Screeing - You must call the Funds office to acquire a voucher before making an appointment with a Vision Screening Provider.
Davis Vision - After making an appointment with a Davis Vision provider, you must contact the Funds office so an authorization can be given before services can be performed.
If you use a provider out of network, you will be reimbursed up to a maximum of $100.00

SASMI BENEFITS
